About the Role
The Manager, Clinical Navigation plays a pivotal role in the daily operations and strategic execution of TailorCare’s clinical navigation program. This leader will oversee a team of Clinical Navigators—empowering them to deliver exceptional engagement, improved outcomes, and a seamless patient experience.
This role blends leadership, clinical expertise, and data-driven insight to ensure our care model consistently delivers measurable improvements in patient outcomes, operational excellence, and patient satisfaction. The Manager, Clinical Navigation will be responsible for driving accountability through analytics, fostering a culture of growth and purpose, and ensuring every patient feels supported and empowered along their care journey.
Primary Responsibilities
Leadership & Team Development
- Lead and develop a team of Clinical Navigators, including senior and lead navigators, to ensure excellence in patient engagement and outcomes.
- Foster a culture of collaboration, compassion, and accountability that aligns with TailorCare’s mission and values.
- Provide mentorship and feedback through regular coaching sessions, performance reviews, and career development plans.
- Promote shared learning, best-practice dissemination, and continuous improvement across teams.
Operational & Clinical Excellence
- Oversee the daily operations of the clinical navigation program, ensuring consistent execution of TailorCare’s care model.
- Ensure compliance with clinical and operational policies, maintaining the highest standards of quality, safety, and regulatory adherence.
- Conduct clinical case reviews and support escalation pathways to ensure sound clinical decision-making and timely care coordination.
- Partner with cross-functional leaders (Training, Quality, Data, Technology, etc.) to refine workflows and elevate program efficiency.
Data, Analytics & Outcomes Management
- Use clinical and operational data to monitor individual and team performance against defined KPIs (engagement, adherence, outcomes, efficiency).
- Translate insights from data into actionable strategies that improve patient outcomes and team productivity.
- Collaborate with the Data , Analytics, and Clinical Stategy teams to identify trends, gaps, and opportunities for improvement in care delivery and engagement.
- Support development and reporting of performance dashboards, ensuring visibility of metrics at both team and leadership levels.
Patient Experience & Engagement
- Ensure that every interaction between navigators and patients is empathetic, evidence-informed, and outcomes-oriented.
- Drive initiatives to enhance engagement, retention, and patient satisfaction throughout their care journey.
- Collaborate with product and care teams to improve communication channels, digital engagement tools, and personalization of patient support.
- Advocate for the patient voice across operational and strategic discussions to continuously evolve the TailorCare experience.
Cross-Functional Collaboration
- Partner with Clinical Operations leadership to align team goals with organizational strategy and growth initiatives.
- Support new market launches, scaling processes, and ensuring operational readiness of the navigation program.
- Represent the navigation team in cross-functional meetings with Product, Strategy, and Operations to influence roadmap priorities and clinical innovation.
Qualifications
- 5+ years of experience as a Physical Therapist (PT); minimum 2 years in a leadership or supervisory role managing teams or clinical operations in musculoskeletal or related fields.
- Graduate from an accredited Physical Therapy (BS, MS, or Doctorate) accredited by the Commission on Accreditation in Physical Therapy Education. Holds one of the following degrees: BS, MS, or Doctorate in Physical Therapy
- Strong familiarity with digital health, remote care delivery, and virtual engagement models preferred.
- Demonstrated success in managing team performance and driving measurable improvements through data.
- Proficient in analytics tools (Excel, dashboarding platforms) and electronic health record (EHR) systems.
- Excellent written and verbal communication skills with the ability to lead conversations with clinicians, executives, and patients.
- Highly adaptable in a fast-paced, evolving environment.
- Current state licensure (TX and GA).
- US work authorization required.
